Through the application of Problem Management, UIT service teams conduct a root cause analysis (RCA) to identify underlying causes of incidents and identify means of fixing or mitigating the impact of a recurrence. The RCA is documented in the Problem Record by the designated SME. For some Stanford Health Care impacting events, and UIT Department Operations Center (DOC) events, RCA reports are created and distributed to organizational stakeholders. The UIT RCA archive is accessible by UIT staff.
